What happened?

Deleting an offering, addon, promotion, component rate, coupling, or witness
removed it immediately on click — no confirmation, unlike product-line
deletion which already prompts. Easy to lose captured work with one
misclick, especially now that cards are dense with data.

Steps to reproduce

Open any provider with an offering/addon/promotion.
Click the ✕ on an offering card.
It's gone immediately, no prompt.

Expected behavior

Same confirm() prompt product-line deletion already uses, scoped to what's
being deleted ("Remove this offering?", "Remove this addon?", etc.)

Environment

Additional context

Fixed in tools/capture-form/index.html — added a scoped confirm() to all
six previously-unguarded delete buttons (offering, component rate,
coupling, addon, promotion, witness). Ready to merge.
