What problem does this solve?

Product lines, offerings, addons, and promotions are all fully expanded at
all times. With a provider that has several product lines each with
several offerings/addons/promotions, the page requires excessive scrolling
to navigate or compare cards.

Proposed solution

Collapsible cards for product lines, offerings, addons, and promotions,
each with a ▾/▸ toggle. Collapsed state shows a one-line summary (e.g. an
offering collapses to "Diamond · $21.99/monthly") so scanning/navigating
doesn't require expanding each card.

Additional context

Implemented in tools/capture-form/index.html. Collapse state is tracked
outside app.state (by stable ID: line_id/offering_id/addon_id/promo_id) so
it doesn't get saved into the provider file or mark the form dirty.
Verified with a headless-DOM test confirming per-card independence. Ready
to merge.
